Before completing the Mediator Selection Form, be sure to:

  1. Choose at least two mediators to serve as primary and alternate mediators on your case (you may wish to choose more than two mediators in the event other lawyers or parties don't agree with your first choices).
  2. Check the mediators' availability to ensure they are currently eligible for assignment to your case.
  3. Submit your Mediator Selection Form within 5 court days of your CMC (Case Management Conference) by either e-mailing the completed Mediator Selection Form to: [email protected] or printing the form and hand-delivering it to the appropriate Mediation Program Office or Civil Business Office Office (see Locations).

IMPORTANT: To email a completed Mediator Selection Form, you MUST use Acrobat Professional or another program or software that can save the information entered on the form (for additional help, click here).
